Support That Fits the Person

No two participants use Creekstone the same way. Some are with us five days a week; others join us two mornings for Group Community Networking or meet a job coach in the afternoons. During intake we'll help you build the right mix, and we'll adjust it as goals change.

Community Integration

Our core service: structured weekdays blending skill-building blocks, activity clubs, and daily community time, with Community Integration support provided one-to-one or in groups of no more than four.

Group Community Networking

Small groups of two to four spend their time entirely in the community: volunteering, errands, the YMCA, the library. They practice life skills where life actually happens.

Employment Readiness & Supported Employment

Job exploration, workplace skills, and one-on-one job coaching at the worksite. We help participants discover what kind of work fits them, then support them to keep it.

Transportation

Door-to-door, wheelchair-accessible transportation across our service area, driven by trained staff who know each rider. Getting here shouldn't be the hard part.

Funding: Creekstone Care is an enrolled provider with the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). Services are funded through Georgia's NOW and COMP Medicaid waivers along with state and federal contracts, service fees, grants, donations, and community partnerships. Contact us to discuss eligibility and funding options. Private pay arrangements are also available, and we're glad to talk it through with your support coordinator.
